About Our Physicians
All Physicians at Colorado Springs
Health Partners, P.C., (CSHP) are board certified or board eligible
in their area of specialty and are fully trained MD's or DO's. When
you register as a patient with CSHP, you will be asked to select a
Primary Care Provider (PCP). Your PCP can be a Family Practice
physician, an Internal Medicine physician, a Pediatrician and/or
sometimes an Obstetrician/Gynecologist. These physicians will
provide most of your health care. When more specialized care or
testing is necessary, you will receive a referral from your PCP to a
specialist. Your insurance plan will many times dictate who can
serve as your Primary Care Provider and when you are able to receive
benefits for specialty care.
Appointments
To schedule an appointment, please
call that office location at the number listed in this site. Be
prepared to explain briefly the nature of your visit so that our
office staff can schedule an appropriate amount of time with your
physician or other clinician. Remember, if you are in a
life-threatening situation, please call 911. For non-life
threatening urgent care, you may call our Urgent Care center
590-1072.

Insurance
If you are covered by an insurer with
whom we do not directly contract, you will need to submit your claim
directly to your insurer. If you are uncertain if your insurer has a
direct relationship with CSHP, please call our Registration
Representative at 475-7700. All questions regarding covered benefits
and rate of reimbursement should be directed to your insurance
company. Please inform CSHP as soon as possible of any changes in
your personal information (name, address, dependents) or in your
insurance coverage.

Referrals
Most insurance plans require that you
obtain a referral from a designated Primary Care Provider (PCP)
before you may see a specialist. If you would like a referral to a
specialist, please make an appointment with your PCP to discuss the
available options. The referral process may take up to 14 days.
Referral authorizations normally have an expiration date. Please
plan accordingly. Typically gynecological exams do not require a
referral from your PCP.
Payment Information for CSHP
Patients
Payment for your care is required at
the time of service. This includes co-payments, insurance
deductibles, co-insurances and self-pay services. Your payment
options include cash, check, MasterCard, Visa and Discover. Please
talk with a Patient Accounts Representative at 475-7700, to discuss
your options if you are unable to pay for service at the time of
your appointment. If your account balance is carried by CSHP for
more than 30 days, you may be assessed a rebilling fee to cover
administrative expenses. Please inform us of any issues as soon as
possible so that we may best address your specific needs.
Patient Rights and Responsibilities
At Colorado Springs Health Partners,
P.C., we believe the following Rights and Responsibilities apply to
all patients of our practice.
Each patient has the right:
- To considerate and respectful
care.
- To obtain from the physician
complete and current information concerning diagnosis, treatment
and prognosis in terms the patient reasonably can be expected to
understand. When it is medically inadvisable to give such
information to the patient, the information should be given to an
appropriate person on the patient's behalf.
- To know the identity and
professional status of individuals providing care to them.
- To reasonably informed
participation in decisions involving his/her healthcare. Except in
emergencies, informed consent should include but not necessarily
be limited to, the specific procedure and/or treatment, the
medically significant risks involved, and the probable duration of
illness or incapacitation. The patient will be given information
concerning appropriate alternative forms of treatment if
available.
- To a safe environment with
suitable privacy.
- To confidential handling of all
communications and records pertaining to his or her care.
- To refuse treatment and to be
informed of the medical consequences of his/her action.
- To receive an itemized statement
for the services rendered by CSHP.
- To change primary or specialty
physicians.
- To expect the highest quality of
care at a reasonable cost.
- To reasonable access to necessary
medical services.
- To expect an answer within a
reasonable period of time to any voiced concerns.
- To obtain a second opinion on
treatment or diagnosis of his or her illness if desired.
Each patient has a responsibility:
- To provide accurate and complete
information about his/her health.
- To ask any questions and seek
clarification as may be necessary to adequately understand his/her
illness and/or treatment.
- To follow the recommended or
agreed upon treatment plan.
- To keep appointments reliably and
promptly and to notify CSHP when unable to do so.
- To fulfill the financial
obligations for care in a timely manner, in accordance with CSHP
policies.
- To show consideration and respect
for the CSHP staff.
