Insurance:
CSHP accepts most insurance plans. Click here to visit our insurance page for more information, and please check with your insurance company or the human resources staff through your employer to determine your specific benefit plan. You may contact our Patient Services staff at 719-866-6575 to find out more about CSHP Primary Care Physicians. Please contact us as soon as possible should your insurance plan change.

Registering as a Patient:
When you call to make an appointment, please let the receptionist know that you are a new patient. You will be asked to arrive 20-30 minutes early for your first appointment to complete the necessary paperwork. Be sure to bring your insurance card with you.
For your convenience, a printable version of the Patient Information Form is available below. Please fill out the form completely, sign and date it, and bring it with you for your appointment. This form is also used for any updates to your information.
