Heart Healthy Living

Heart healthy living is an education program designed to assist people who would like to better manage their cardiovascular risk. Conducted by Nurse Educators, Dietitians, and Physicians, our Heart Healthy program is composed primarily of group education sessions.
During group sessions, you will:

  • Learn about cardiovascular disease
  • Understand heart attack warning signs
  • Identify cardiovascular risks and your own personal risks
  • Learn about choices that are healthy for your heart, such as modifiable risk factors, food guide pyramid, cholesterol, fats, and their effects on the body
  • Learn about the role of cholesterol medications
  • Visit with a Clinical Lipidologist
  • Establish an action plan with one-on-one help from a Physician and dietitian
  • Identify and track progress


A physician referral is required to enroll in this program. Prior to your first session, the Nurse Educator and Registered Dietitian will begin to develop a personalized action plan for you based on lab results and notes from your PCP.

Individual Lipid Consultations

If you cannot tolerate cholesterol medication, have high cardiovascular risk, or have multiple lipid abnormalities, you may benefit from an individual consultation with a board-certified Clinical Lipidologist. CSHP has two physicians, Kari Uusinarkaus, M.D. and Stacey Porterfield, D.O., who have achieved this advanced certification.
Each consultation begins with a 30-minute initial visit, where you will cover topics such as:

  • Medical evaluation from a cardiovascular risk perspective
  • Diet and exercise recommendations
  • Lifestyle tips for improving risk factors
