Health Management

	CSHP’s Health Management department is comprised of a highly qualified team of educators and medical staff. Programs provide easily understood, in-depth teaching about diseases with practical recommendations. We are committed to helping patients make changes that will improve their overall health, and enable them to lead healthier lives. Our programs provide:
	Consistent, up-to-date, and in-depth education Communication during enrollment with both the participant and their primary or referring medical provider Participant accountability in participation and completion of the program Tools to be successful in lifestyle changes Group and one-on-one attention for ongoing support Networking opportunities with other participants
